The Delaware Division of Public Health is conducting a survey of parents and caregivers of children and youth (0-25 years of age) with special health care needs. This survey hopes to find out about your children’s health care needs, and your experiences with the services that are available to support those needs.
DPH wants to ensure that parents of children and youth with special health care needs have access to support services, especially as it relates to health. This includes children with conditions like asthma, sickle cell disease, autism, attention deficit disorder, depression, allergies and many others. Your input is critical to improving health and services for children like yours in our community.
Your participation in this survey is voluntary. The survey is completely anonymous, all results will be grouped, and NO personal identifying information will be collected.
You are free to decline any question you do not wish to answer. The survey should take between 15 and 20 minutes online. The online survey will close on April 14, 2015. We hope that you will be able to find a few minutes to complete the survey!
